Accident Summary:
At 0845 hours, 28 Dec 68, the commanding officer of the 155th Assault Helicopter Company Drove Major McDonald to his aircraft. At this time, he checked the aricraft's fuel quantity gauge in order to ascertain that his POL section had refueled the aircraft. He noted that the aircraft had 1,200 pounds of fuel. He then bid farewell to Major McDonald and returned to his orderly room. At 0912 hours, 28 Dec 68, Maj McDonald called for departure instructions from the north SOD, Ban Me Thout City Field. The aircraft took off in a northeasterly direction over a rubber tree plantation. The aircraft had travelled approximately 1,000 meters from takeoff when it crashed.
